PROSTATE CANCER SCREENING (G0103) |
|
Screening prostate specific antigen (PSA) tests are covered at a frequency of once every 12 months for men who have attained age 50, if at least 11 months have passed following the month in which the last Medicare-covered screening prostate specific antigen test was performed. A screening PSA is a test that measures the level of PSA in an individual’s blood. PSA is a reliable immunocythochemical marker for primary and metastatic adenocarcinoma of the prostate. This screening test must be ordered by the beneficiary’s physician or by the beneficiary’s physician assistant, nurse practitioner, clinical nurse specialist, or certified nurse midwife. In addition, the provider ordering the screening test must be fully knowledgeable about the beneficiary’s medical condition, and would be responsible for explaining the results of the test to the beneficiary. |
This policy has been duplicated from the most current Medicare National Coverage Determination or Local Medical Review Policy available at the time this booklet was printed in October 2005. Medicare will consider this test(s) medically reasonable and necessary only if the test(s) was performed for any of the diagnoses/conditions listed on this page. This information is subject to change without notice, and Covenant Health Care Labs (CHC) is not responsible for consequences that may result from using such information. CHC is not indicating one code is more acceptable than another, and that selected codes should reflect the patients actual medical condition as noted in their medical record.
